The Blue Screen Occurs After the System Restart upon Installation of the SFWHA5.x Software

Issue Description

If the IBM SDDDSM or RDAC multipathing software and the SFWHA5.x are installed on a low-end or mid-range IBM disk array such as the DS4000, the blue screen occurs after a system restart. 
1. The system log indicates that the blue screen code is 0x0000007E.
2. According to the blue screen information, the ds4dsm.sys driver caused the blue screen.

Handling Process

Uninstall the IBM SDDD SM or RDAC multipathing software, and then install the Veritas DMP DSM multipathing software.

Root Cause

This problem is caused by the IBM SDDD SM or RDAC multipathing software. According to the Hardware Compatibility List (HCL) of the Veritas Storage Foundation (tm) and High Availability Solutions 5.0 &5.1 (SFW/SFW HA), the SFWHA supports only the Veritas DMP DSM multipathing software, not IBM multipathing software.
