The customer is having issues with an AP4030DN that remains in fault state.
AC version: V200R007C20SPC300 / AP version V200R007C20SPC500
The AP is connected directly to the AC interface G0/0/1
From the output of display ap all shows the ap with the correct mac-address in fault state.

1. Requested from customer the output of display diagnostic-information.
a. The configuration was very simple capwap source is vlanif1 and also the dhcp was selected on the vlanif1 interface
b. Checked the status of the AP and it appeared in fault state.
c. Checked the output of display ap offline-record and we did not find any records.
2. Requested from customer to gather trace of the AP while the AP is booting up to check the messages between AC and AP.
a. In order to do the trace I informed customer to do the following steps:
i. <AC>Trace enable
ii. <AC>trace object mac-address 2c55-d395-4900 ( mac-address of AP )
iii. To turn off the trace use the <AC>undo trace enable

From the trace we noticed that the issue was related to an image update failure.
The reason the AP is in fault it’s because in the configuration of the AC you have configured “ap update update-filename FitAP4X30XN_V200R006C20SPC800.bin ap-type 43 ap-group default” and the file does not exist in the flash of the AC and because the upgrade fails the AP remains in fault state.
In order to solve this issue we used the undo ap update update-filename ap-type 43 ap-group default command and rebooted the AP.
